Services for the transportation of the national youth football team of Ukraine U-19 according to the route: from the city of Kyiv (UAF, Laboratorniy ave., 7A) to the city of Warsaw (F. Chopin Airport) via the city of Lviv and from the city of Warsaw (Modlin Airport) via the city of Lviv to the city of Kyiv (UAF, ave. Laboratory, 7A). 08.11.2025 - transfer from Kyiv (UAF, Laboratorniy Ave., 7A) to Warsaw (F. Chopin Airport) via Lviv; 11.18.2025 - transfer from Warsaw (Modlin Airport) via Lviv to Kyiv (UAF, Laboratorniy Ave., 7A). Requirements for carrier: 1. fill in the information provided in the purchase annexes; 2. providing a contract that will confirm the provision of similar services for the past two years (from the date of publication of the procurement); 3. has been operating for more than 1 year (from the date of publication purchases) 4. availability of a license for international transportation of passengers; 5. the vehicle to which the services will be provided must be owned or leased by the carrier; 6. scan copy of the vehicle registration certificate; 7. photo of the vehicle (from the outside, from the middle and the luggage compartment), which will be provided services; 8. in the case of involvement of third parties in the provision of services under the contract - to fulfill the requirements specified in points 4-7 in relation to such third parties.
